Report of the Panel on Human FactorsHuman error in the operation of complex systems is the largest single cause of incidents, accidents, and the loss of lives and dollars. Therefore, it is understandable that the focus of human factors research and engineering is on increasing our understanding of the basic mechanisms of human error and on developing techniques for reducing or eliminating the various causes of human error.
