The deadly triadThe contributions of a design deficiency, supervisory shortcoming, and pilot error to the occurrence of a major accident involving a mechanically intact fighter aircraft during VFR weather are analyzed. The need for physiologically oriented flight surgeons to exercise critical judgment about the adequacy of the man-machine interface is pointed out.
