Management of human error by designDesign-induced errors and error prevention as well as the concept of lines of defense against human error are discussed. The concept of human error prevention, whose main focus has been on hardware, is extended to other features of the human-machine interface vulnerable to design-induced errors. In particular, it is pointed out that human factors and human error prevention should be part of the process of transport certification. Also, the concept of error tolerant systems is considered as a last line of defense against error.
